I agree to pay the full tuition amount by the start of the camp season. I understand that failure to pay may result in my child being unable to attend.

I grant permission for my child to be photographed or videotaped during camp activities. These images may be used for camp promotional materials, social media, and the camp website.

In case of emergency, I authorize camp staff to seek medical treatment for my child. I understand that I will be responsible for any medical expenses incurred.

AUTHORIZATION PERMIT: In case of emergency, illness injury, I hereby authorize personnel of Camp Fun—Monsey Camp Program or the doctor/hospital to which my child may be brought (and whomever they may designate as their assistants) to perform an emergency procedure or operation, to give treatment injections and administer an anesthetic to my child. Swimming Authorization: I hereby give my children permission to go offsite swimming on all trips, and swimming including but not limited to the Visions Pool on daily basis. Lost and Found Policy: All articles left in camp after the last day will be considered part of the Camp fun Tuition. Waiver of Liability: I release Camp Fun, Monsey Camp Program and anyone affiliated with or third party under their direction or properties leased by us from any legal liability resulting from injury or loss to my child. Monsey Camp Program is under permit with the Rockland County Dept. of Health and is inspected twice a season. Copies of the inspection reports are on file in the camp office. HOLD HARMLESS AGREEMENT CAMP ACTIVITIES, TRANSPORTATION AND TRIP WAIVER For good and valuable consideration, receipt of which is hereby acknowledged I do hereby covenant and agree to defend, indemnity, and hold harmless Camp Fun Monsey Camp Program, Mordechai Schwab, from and against any and all liability, loss, damages, claims, or actions, including costs and attorney fees for bodily injury to the extent permissible by law arising out of or in connection with the actual transporting of campers; Camp Ora, Camp Fun, Monsey Camp Program, and any other transportation and all camp activities and all camp trips including but not limited to: I hereby give my child(ren) permission to go on the camp trips during & after camp which may include but are not limited to Boulder Stadium, Camel Beach, Mountain Creek, Bounce Trampoline Sports, Banana Boating, Top Golf, White Lake, 94-96 Wilder Road, Playland, KJ Park, American Dream, Niagara Falls trip, Delaware rafting and camping, motor boating and tubing, Keansburg Water Park, Kalahari, KJ Park, Yankee game, American Dream, Niagara Falls trip, Inflatables, and all camp activities involving the camp both on and off the camp premises. I give permission to all the camp fun/ora staff to take and distribute pictures and videos of my children in all camp related activities. This also will include family, relatives, guests, and invitees. THE UNDERSIGNED IS OVER 21 YEARS OF AGE AND HAS READ THIS FORM AND AGREES TO COMPLY WITH SAME. TO THE BEST OF MY KNOWLEDGE, ALL OF THE ABOVE INFO IS CORRECT. I HAVE READ AND ACCEPTED ALL THAT IS WRITTEN ABOVE. I HAVE READ AND AGREE TO THE CAMP FUN TERMS AND CONDITIONS OF THE REGISTRATION FORM.
